    （一）当事人是公民的，应当查明双方的姓名、性别、年龄、籍贯、住址和联系电话；当事人是单位的，应当标明单位的名称、地址、联系电话和法定代表人或者负责人的姓名。

    （1） If the party concerned is a citizen, the name, sex, age, native place, address and telephone number of both parties shall be ascertained; If the party is a unit, the name, address, contact telephone number and the name of the legal representative or person in charge of the unit shall be indicated.

    （二）证明民事法律关系发生、变更、消灭等事实的证据：如合同、协议、债权文书（IOU、IOU等）、收发凭证、信件等；

    （2） Evidence to prove the occurrence, change and elimination of civil legal relationship, such as contract, agreement, creditor's right document (IOU, IOU, etc.), receipt and delivery certificate, letter, etc;

    （三）确定债权、债务数额的证据；

    （四）证明诉讼主体资格的证据：证明原告资格的材料的原件和复印件，如居民身份证、户口登记簿、护照、香港同胞和澳门同胞的回国证明、结婚证书等证据；企业为原告的，应当提交营业执照、工商登记证等材料的复印件；事业单位应当提交事业单位法人代码证；原告为银行、非银行金融机构的，还应当提供金融许可证。证明被告主体资格的材料复印件，如营业执照、身份证、护照等；被告是在工商行政管理机关登记的企业的，应当提交最近的工商登记表。被告人姓名变更的，还应当提供变更登记表；

Copies of materials proving the defendant's subject qualification, such as business license, ID card, passport, etc; If the defendant is an enterprise registered with the administrative department for Industry and commerce, he shall submit the latest industrial and commercial registration form. If the name of the defendant is changed, a registration form for the change shall also be provided;
